If you are a self-funder, you can refer yourself or the person you care for to our Day Service or Supported Living service online, without waiting for a paper form.
Day Service & Supported Living
Our Day Service at 150 Watnall Road gives adults with mild to medium learning disabilities somewhere safe, friendly and full of life to spend their days, with activities, friendships and support built around them.
Our Supported Living service, further along the same road, helps people live as independently as possible with the right support in place.
This self-referral form is for people paying for their own care (self-funders). If your care is arranged or funded by the local authority, please speak to your social worker, or contact us and we will point you in the right direction.
How a self-referral works
- Fill in the online form with the applicant's details, the service you are interested in, and as much detail as you can about their needs.
- We email you a link to confirm your email address, so you can sign in and check on your referral at any time.
- Our team looks at what you have sent, and will be in touch to talk through next steps.
The form takes about ten minutes. You can sign in afterwards to see how your referral is getting on.
